New Patients


Thank you for choosing Taylor Family Practice for your medical care. This page is intended to help new patients become familiar with our practice and our office policies. 

Appointments

If you have an urgent issue, we can usually accommodate you the same day or by the next business day. If you have an urgent problem after hours or on weekends, please go to your nearest emergency room or call 911.

Routine appointments will be scheduled as openings are available. We request that you give us at least 24 hours notice if you are unable to keep a scheduled appointment so that we may offer it to another patient in need.

Prescription Refills

All prescription refills will be called in during office hours. We do not call in refills after hours or on the weekends. Please give us at least 48 hours notice when you need a refill. This allows us time to review your medical chart and contact the pharmacy or notify you of the need for an appointment if that is the case. Please note that no refills will be phoned in for narcotic medications.

Release of Medical Records

We would be more than happy to release your medical records to another health care provider when needed. However, we do require that a medical records release form be signed. You can contact our office for this form. Please allow 3-4 business days for processing this request when possible.

Payment Policy

If you have insurance with a health plan for which we are contracted, we will file your insurance for you. You will be responsible for your co-payment or any deductible at the time of your visit. For your convenience, we do accept Visa and Mastercard (Debit and Credit) and we also accept checks with a valid photo id.

If you do not have health insurance and need to arrange a payment plan, please contact our office and ask for the office manager.
